How will an ambulatory care pharmacist or other allied health provider including in specialty clinics know a Primary Care Clinical Pharmacist (PCCP) has seen a patient?
- A MR-PCN code in a patient’s PharmaNet profile indicates that a PCCP has provided care to that patient.
How can ambulatory care pharmacists and other allied health providers and PCCPs work together?
- When a MR-PCN code is in PharmaNet, the hospital pharmacist can request documentation or discuss handover with a PCCP.
- Some PCNs permit direct referral from a hospital pharmacist to a PCCP or recommend a PCCP referral to family physician/nurse practitioner.
