Pharmacists Aligned in Shared Care Teams (PACT)

The Pharmacists Aligned in Shared Care Teams (PACT) program is intended to optimize shared patient care between pharmacists who, through a combination of their work setting, motivation, and patient panel, expect to collaborate with PCCPs or other pharmacists in the care of mutual patients.

The PACT program is being administered by Continuing Pharmacy Professional Development (CPPD) and Parts 1 and 2 are now available. Learn more about the course here. You can enrol for the course here
